End of Life Organization Workshop

| FreeJoin Oakwood Cemetery and Stacy Jurgens for an informative discussion on the Five Wishes document. Five Wishes was the first advance directive to address personal, emotional, and spiritual issues in addition to meeting medical and legal criteria.
Attendees will receive a copy of the five wishes, the nation’s only national advance care planning program, “Providing peace of mind for 25 years”. Five Wishes was designed to be accessible, legal, and easy-to-understand with the goal of helping people discuss and document their wishes in a non-threatening, life-affirming way. When complete and notarized, The Five Wishes document is an advanced directive and accepted nationally at hospitals and end of life care programs as a testament to your desired treatment.
Stacy Jurgens, M.A. in psychology and more than 10 years of social services experience in skilled nursing facilities and end-of-life care units working through this document with residents and their families; she is passionate about working with people to ensure they experience their highest quality of life on their own terms.
